These Terms and Conditions (“Terms”) govern the purchase and use of the CareTag.uk service by individual consumers. By purchasing a CareTag profile or using the CareTag.uk website (the “Website”) and services as an individual user, you (“you” or “User”) agree to be bound by these Terms. The Service is operated by Health Pros Network Ltd, Company No. 15970093, registered in England and Wales, with registered office at 3rd Floor, 86-90 Paul Street, London EC2A 4NE, UK (referred to as “CareTag”, “we”, “us” or “our”). “CareTag.uk” refers to our Website and related services for hosting emergency contact and medical information profiles.
Please read these Terms carefully before using the Service. If you do not agree with these Terms, you should not purchase or use the CareTag.uk service. We recommend that you print or save a copy of these Terms for your records (you can also find them on our website at any time). These Terms do not affect your statutory rights under UK consumer law.
CareTag.uk provides a service where individuals can store personal and medical information in an online profile accessible via a secure, unique URL or NFC-enabled wristband or wallet card.
Key points about the Service:
You must be 18 years of age or older to purchase the CareTag.uk service for yourself. If you are under 18, a parent or legal guardian must purchase and consent on your behalf. Individuals under 13 are not permitted to use the Service directly; a parent/guardian must set up the profile for any child under 13 (with parental consent). By agreeing to these Terms, you confirm that you meet these age requirements or are a parent/guardian acting for a minor.
To use the Service you will complete our order form (currently via Typeform or directly on the Website), providing the required information and any optional medical details you choose to include. You agree to provide accurate, current, and complete information. Providing false or misleading information is prohibited and could be dangerous in an emergency.
During signup you give explicit consent for us to process any health/medical information you provide. You can withhold or later withdraw consent, but without it we cannot include that information in your profile.
If setting up a profile for a child or minor under your care, you confirm you are the parent or legal guardian with authority to consent to these Terms and the processing of the child’s data.
Profile edits are currently handled by contacting our support team (support@caretag.uk). Each purchase creates one profile for one individual only.
You agree we may contact you by email (or phone if provided) for essential service communications (order confirmation, expiry reminders, support, etc.).

You acknowledge that CareTag.uk is not a substitute for professional medical advice or treatment.
The price shown at checkout (£39.99 including free UK shipping) is a one-time, non-recurring payment that covers the physical kit and 5 full years of profile hosting. No further payments will ever be requested or taken.
Orders are placed via our Website or Typeform link. Your order is an offer; we confirm acceptance by email after successful payment, at which point a binding contract is formed. Payment is processed securely via Stripe.
Physical items are dispatched promptly (normally within a few working days). Your profile is activated within 1–2 business days of order confirmation.
5.1 DurationYour CareTag profile is active for a fixed period of 5 years from the activation date.
5.2 End of the 5-Year Period60–30 days before expiry we will send a courtesy reminder to the email address you provided. If you take no action, the profile and all associated data will be permanently and irreversibly deleted 30 days after the 5-year anniversary. The NFC wristband/wallet card and unique link will then cease to function.
5.3 Early Deletion (Your Right)You may request immediate deletion of your profile at any time by emailing support@caretag.uk with the subject “Delete my profile”. We will delete everything within 24 hours (usually much faster).

We may terminate or delete your profile early only for serious breach of these Terms. We will always attempt to contact you first.
You have a legal right to cancel within 14 days of contract formation without giving a reason.
Because we begin providing the digital service immediately, if you cancel within 14 days we may deduct a small pro-rata amount for the days the profile was active (usually only a few pence). We will clearly state any deduction before processing your refund, which will be issued within 14 days using the original payment method.
To cancel, simply email support@caretag.uk with a clear statement. You do not need to use a special form.
After the 14-day cooling-off period the purchase is non-refundable because it is a one-time payment for 5 years of service delivered upfront.
Faulty or damaged physical items will of course be replaced free of charge or refunded under the Consumer Rights Act 2015.
Anyone who scans your CareTag or obtains the unique link can view the information you chose to include. We make all reasonable efforts to keep the link private and unguessable, but ultimate responsibility for the physical item rests with you.
We do not guarantee that first responders will recognise or use your CareTag, nor can we be liable for medical decisions or outcomes. The Service is a passive information tool only.
Nothing in these Terms excludes or limits our liability for death/personal injury caused by negligence, fraud, or any matter that cannot lawfully be excluded.
To the fullest extent permitted by law, our total liability to you is limited to the amount you paid us (£39.99). We are not liable for indirect, consequential, or special losses, or for events outside our reasonable control.
Your data is processed in accordance with our Privacy Policy (available on the Website). By purchasing you confirm you have read and accept it.
We may update these Terms from time to time. Material changes will be notified by email or prominent notice on the Website.
These Terms are governed by the laws of England and Wales. The courts of England and Wales have non-exclusive jurisdiction (Scottish and Northern Irish consumers may also bring proceedings in their own jurisdiction).
These Terms together with the Privacy Policy constitute the entire agreement between you and us.
If any provision is found unenforceable, the remainder continues in full force.
